Preparing for a medical exchange
A clear chronology and full reporting help explain a situation. This sheet is used to prepare your questions; it does not choose any diagnosis, test or treatment.
Your preparation sheet
Notes remain in the fields on this page. This tool neither sends nor saves them. Avoid unnecessary identifiers. Closing or refreshing may erase them; your browser may restore fields.
A useful chronology example
Fictitious example: "Return of travel Monday; first signs Tuesday evening; sampling Wednesday; result received Thursday."This distinguishes possible exposure date, beginning of signs, sampling and receipt of the result. This reconciliation does not prove the cause of the signs.
During the exchange
Start with the main motive and priority issues. Present the original documents if possible. Make clear the instructions you have not understood and note the planned follow-up with the professional.
