Situation and vulnerability
Skin contacts with infected lesions and small injuries allow transmission. The remoteness of care can facilitate complications.
Signs and limits of their interpretation
An embossed injury may sulcer. Multiple lesions or bone pain and swelling may occur afterwards.
Diagnosis and monitoring
Serology requires careful interpretation; a PCR on lesion can confirm the pian. Some similar ulcers are caused by other bacteria.

Prevention and care
The disease is treated by prescribed antibiotics. Contact tracking and follow-up are part of the health programmes; a persistent ulcer must be evaluated.
Practical issues
Does a treponemic serology always distinguish pian and syphilis?
No: the usual tests do not automatically distinguish them. The clinical and epidemiological context as well as the appropriate examinations are essential.
